Office Policies

Thank you for choosing Revive Med Spa. We love that you have chosen us to take care of your aesthetic & wellness needs. We value you as a client and hope to help you with your future endeavors; however, we can only accomplish this with your understanding of your obligations set forth in this policy and consistency with treatment. Thank you for your understanding and for choosing us!

Although we encourage you to complete all forms before arriving, If you have not completed the online forms, it would be advisable to arrive 15 minutes before your appointment to complete the necessary forms.

 

Early Arrival

You are welcome to arrive early to have a cup of coffee or simply to find a tranquil setting before returning to your hectic life at home, but please keep in mind that arriving early does not guarantee that your service will begin before your scheduled appointment time. Please call ahead to check if your service can begin earlier than your scheduled time.

 

Cancellation Policy

Your treatment at Revive Med Spa is reserved exclusively for you. Please notify any cancellations 24 hours (1 business day) prior to the appointment date. All cancellations with less than 24 hours’ notice are subject to a $25 cancellation fee.

 

No Show / Late Arrival

We do our best to accommodate, but please be mindful that treatment cannot be guaranteed if you arrive more than 15 minutes late. A “No Show” is someone who misses an appointment without canceling it at least 24 hours in advance. In the event a 24-hour notice is not given, a fee of $25 will be charged or must be paid prior to rescheduling future treatment.

 

Complimentary & Consultation Services

Complimentary services and consultation services – No-shows or same-day cancellations that are less than 12 hours before your appointment time, there will be a $35 Consultation fee

 

Frequent Cancellations Or Missed Appointment

Clients who cancel, reschedule, or arrive late frequently (3 or more in 6 months or 2 consecutive) will be required to schedule their appointments on the day they want services, rather than being offered the ability to schedule in advance.

 

Credit Card Hold Policy

Services, require a credit card to reserve your appointment. If you book online you will receive a phone call for credit card authorization to hold your appointment time. **Your credit card will not be charged and you may pay in another method at the time of your appointment. **Charges on your credit card will only occur based on the cancellation policy above.

 

WHEN THE NO-SHOW/LATE CANCEL FEE IS WAIVED

The only exception to this cancellation policy is in the event of a serious or contagious illness or emergency. We offer one grace for these purposes every six months. Some examples of emergencies are car accidents, deaths in the family, or extreme illness. Work issues do not constitute emergencies.

 

Refund Policy

Aesthetic results are quite variable from person to person and while we do our best to achieve the desired outcome it cannot always be guaranteed. Clients are responsible for further treatments needed to achieve further results. Please be aware that we are unable to make any refunds for any product or service that has been purchased, injected, or used in your treatment. They can be, however, exchanged for other products or treatments of equal value. Also, no exchanges are allowed 2 weeks after the purchase has been made, except in the cases of a medical illness or pregnancy. The exchange value will be calculated by subtracting the pro-rated value of all services rendered from the purchase amount. Gift Cards – Unused gift cards don’t expire for five years However, if you don’t use your card within a year, there is a 30% “inactivity fee.” For all purchases made via Groupon, all refunds must be requested through them. Revive Med Spa is not responsible for Groupon refunds. We do not offer refunds on products purchased. However, should you have an adverse reaction to one of the Products, it can be returned for a full refund within 7 days of purchase. In the case of an allergic reaction: all reactions must be reported within the first 48 hours of use. The Revive Med Spa reserves the right to request photos of the reaction and/or a doctor’s note to confirm the reaction and that it was caused by the use of our products.
Defective products (i.e., a broken pump) may be exchanged within 7 days for the same product only.

 

Environment

Please be aware that we maintain a tranquil and relaxing atmosphere within our premises and would like to remind you to be respectful of the other guests’ right to privacy and serenity during treatments. Please keep conversations to a minimum and at a considerate volume while also avoiding phone conversations and other noises that may disrupt the tranquility of others.

 

Personal Belongings

Please note that you are always responsible for your own belongings and that Revive Med Spa is not responsible for any lost or damaged items.

 

Gratuities

The industry standard for gratuities is between 15-20%, however, this depends entirely upon you and your level of satisfaction with the treatment. It is at your discretion and always appreciated.

 

Gift Certificate & Gift Card Policy

Gift Certificates and Gift Cards are non-refundable and cannot be redeemed for cash or gratuities. and cannot be replaced if lost or stolen.

 

Payment

Payment for all individual treatments must be done at the time of the treatment and all packages must be paid in full at the time of the first treatment. Please note that some treatments extend over a period of time. There are no refunds or credit issued for any service that include but aren’t limited to; laser hair removal, IPL, Botox, Fillers, Facials, and Massage. As our treatments are limited to elective aesthetic medicine, we do not bill insurance. We accept cash, personal checks, Visa, MasterCard, Discover, and American Express. All prices are subject to change without notice.

 

Privacy Policy

At Revive Med Spa, we take privacy seriously. We ensure that the staff does not discuss client treatments with any other clients and that your privacy is maintained at all times. As a courtesy to our patients our systems, automatically email appointment reminders 48 hours prior to the scheduled date. If, however, you choose not to share your email address and phone number we will be unable to provide you with a reminder.

 

Practice-Patient Relationship

We reserve the right to refuse service at any time, to anyone, for any reason. We reserve the right to refuse appointments to any client who has demonstrated disregard of policies.

 

Quick Summary Of Policies

You cannot undergo any treatment in any of our offices if you are pregnant (or think you may be), nursing, undergoing fertility treatments, or preparing to undergo fertility treatments.
Please notify us if you have any medical conditions or history such as heart disease, thyroid disorder, diabetes, cancer, skin disease, or anything else that we may need to be aware of.
If you are under the age of 18, you must arrive at your appointment with a parent or legal guardian to receive laser hair removal or skin tightening services.
Minors are not eligible to receive injections, Morpheus8, Evolve, and Forma services.


The practice of medicine is not an exact science and no guarantees can be made concerning the results of such procedures; therefore, there will be NO refunds for any treatments received.

Please note all pre-paid packages are non-refundable and non-transferable. All deposits are non-refundable, non-transferable, and may not be transferred to be used for a different service than what was originally intended.
